Some clients in supported living may need only the services on an in-home support worker or only the services of a supported living coach.<\/li>\n \t<li style=\"text-align: left;\">Other clients may need both services. When both services are used, the providers will coordinate their activities to avoid duplication. Reimbursement and monitoring documentation to be maintained by the provider:\n<ul>\n \t<li>Copy of claims submitted for payment<\/li>\n \t<li>Copy of services log<\/li>\n \t<li>Monthly summary notes<\/li>\n \t<li>Staffing documentation<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2 class=\"\" style=\"text-align: center;\"><span style=\"color: #3c424f; font-weight: bold; font-family: 'times new roman', times, serif;\">Supported Living<\/span><\/h2>\n<ul class=\"\" style=\"text-align: left;\">\n \t<li>\u00a0provide training and assistance to support consumers in a variety of activities that are necessary to establish, live in and maintain a household of their choosing in the community.<\/li>\n \t<li>These services may include assistance with locating appropriate housing, the acquisition, retention or improvement of skills related to activities of daily living\n<ul class=\"\">\n \t<li>such as personal hygiene, grooming, household chores, meal preparation, shopping, personal finances and the social and adaptive skills necessary to enable consumers to reside on their own.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3 class=\"\" style=\"text-align: center;\"><span style=\"font-weight: bold; font-style: italic; font-family: 'times new roman', times, serif;\">The supported living coach will:<\/span><\/h3>\n<ul class=\"\">\n \t<li style=\"text-align: left;\">Complete a Functional Community Assessment, the basis for identifying the types of training, assistance and the intensity of support, shall be completed with the consumer prior to his\/her move to a supported living arrangement and updated annually.<\/li>\n \t<li style=\"text-align: left;\">Complete a Housing Survey, the basis for surveying a prospective home will be completed and forwarded to the support coordinator within ten (10) working days of the selection and will be updated quarterly. This update shall include a review of the beneficiaries\u2019 overall health, safety and well-being status.<\/li>\n \t<li style=\"text-align: left;\">Will assist the consumer in completing a Financial Profile analysis and submit it to the support coordinator for no more than 10 days following the selection of housing by the consumer. The financial profile is an analysis of the household costs and revenue sources associated with maintaining a balanced monthly budget for the consumer. The analysis will substantiate the need for a monthly subsidy or initial start-up cost and will be a source of information for determining strategies for assisting the person in money management.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<\/div>","protected":false},"excerpt":{"rendered":"<p>Services Services are provided within the guidelines laid out in the Medicaid Waiver Handbook.
